Ohio’s expansion group, by the numbers
Ohio calls its Medicaid expansion adults Group VIII: adults 19 to 64 with income under 138% of the poverty level, $22,025 for one person, who don’t qualify for Medicaid another way. In July 2025 that was 774,342 Ohioans.
Above 138%, a Marketplace tax credit usually takes over, up to 4 times the poverty level.24
Work rules: Ohio wrote its own first
Who it affects: Ohio Medicaid expansion adults aged 19 to 64.
Ohio passed its own work requirement for the expansion group in House Bill 33 in July 2023 and asked CMS to approve it in February 2025. The federal law passed in July 2025 now requires work rules for expansion adults nationwide, and Ohio is aligning the two.
Ohio’s own count shows most of the group already meets the rules through work, age or a medical exemption. About 1 in 5 will need to be assessed.2
A family of three
For a family of three, Ohio Medicaid for the adults reaches about $37,700 a year, and children qualify up to about $57,600.45
